The notices below describe your rights as a Doorstep patient. The HIPAA Notice of Privacy Practices and the Good Faith Estimate notice are required by law.

HIPAA Notice of Privacy Practices (NPP)

This Notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our duties

We are required by law to protect the privacy of your protected health information (PHI), to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. If we change this Notice, the new terms will apply to all PHI we maintain, and we will post the updated Notice on this website.

How we may use and disclose your PHI

We may use and share your PHI without a separate authorization for these purposes:

  • Treatment — to provide and coordinate your care, including sharing information with the clinician who treats you and with other providers involved in your care.
  • Payment — to bill and collect for your care, including preparing your Good Faith Estimate and, for personal-injury cases, coordinating lien or Letter of Protection documentation and case funding.
  • Health care operations — to run and improve our practice, such as quality review and clinician training.

We may also use or disclose your PHI when required by law, for public health and safety, to comply with legal process, or as otherwise permitted by the HIPAA Privacy Rule. Most other uses — including most marketing and any sale of PHI — require your written authorization, which you may revoke at any time.

Your rights

You have the right to:

  • see and get a copy of your records;
  • ask us to correct information you believe is wrong or incomplete;
  • ask us to limit how we use or share your information;
  • ask us to contact you in a specific way or at a specific place;
  • get a list of certain disclosures we have made;
  • get a paper copy of this Notice, even if you agreed to receive it electronically; and
  • be notified if there is a breach of your unsecured PHI.

How to file a complaint

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Good Faith Estimate notice

Your right to a Good Faith Estimate of expected charges under the federal No Surprises Act.

You have the right to receive a Good Faith Estimate explaining how much your care will cost.

Under the law, health care providers need to give patients who don't have insurance, or who are not using insurance, an estimate of the expected charges for medical items and services. Because Doorstep is a self-pay practice and does not bill insurance, this right applies to our patients.

Your right to a Good Faith Estimate

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as your evaluation, treatment visits, and any equipment or supplies.
  • You can ask Doorstep, or any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you schedule care, Doorstep will give you a Good Faith Estimate in writing at least 1 business day before your appointment.
  • Make sure to save a copy or a picture of your Good Faith Estimate.

What your estimate includes

Your written estimate lists the items and services reasonably expected for your course of care — for example, your initial evaluation and the treatment visits we anticipate — with the expected charge for each. Because care is tailored to your needs, the estimate reflects what we expect at the time it is prepared; your actual care may differ.

If your final bill is higher than your estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal patient-provider dispute resolution process. You must start the dispute process within 120 calendar days of the date on your bill. Disputing a bill does not affect the quality of care you receive.

How to request a Good Faith Estimate

Email us at hello@movewithdoorstep.com to request a Good Faith Estimate and we'll provide one in writing. We also provide one automatically before any scheduled care.
