Clinical capabilities
Our state-licensed physical therapists provide comprehensive outpatient rehabilitation in the patient's home, including post-surgical rehabilitation, orthopedic and musculoskeletal care, balance and fall-risk management, and recovery from accident-related injuries. Each patient receives a full initial evaluation and an individualized, goal-oriented plan of care.
Communication process
We send an evaluation summary after the initial visit and progress updates at the intervals you prefer, with a discharge summary at completion. We are responsive to your office's documentation requests and welcome direct clinician-to-physician contact when it benefits the patient.
How to send us a patient
Use our referral form, or have your office fax or send the order with the patient's contact information. We handle scheduling directly with the patient and confirm receipt of every referral.
